Just for the record; I'm not a plant expert either. Just an opinionated old codger who thinks if you need to know where it came from you don't know what it is.

Human hands very quit a bit in size and do not make a good scale reference.

Please use a ruler for scale.

To be fair this information is quite necessary to identify things at the species level. Something that I rarely care about.

Hands are a poor measuring tool, but sometimes we work with what we have. ;) 

Looks like a decent piece of Lepidodendron sp. bark.

As for the fossil I second @Rockwood albeit this does look like an impression rather than the actual fossil. Still a neat find.

No. Technically speaking it is an external mold fossil. 

Trace fossil are the result of the activity of a life form, not the existence.
