There are two views from above (top view) and two from the sides. The "base" is sandstone.

Yes, this is from the Port Jervis Formation in Cuddybackville NY.

Definitely Port Jervis (Trilobite Ridge) Formation. I didn't know the Port Jervis Formation was exposed in Cuddybackville. Great find! Congratulations, Gordon and thanks for sharing.
