Fork glued, but probably just removed (and reattached) to prep it freestanding (although sometimes a flying fork is a sign of composition), it's normal. Prolly took an experienced preparator 20-30 hours just to prep, now multiply that with the rate of your dentist (and that's just a part of the costs behind a bug like this).
