Compare with Isurus paucus.

I don't know much about these sharks but it was the first thing that came to mind as they have that notch in the blade.

 I will add, since I just remembered the other name for Isurus paucus is Isurus retroflexus. 

Also Isurus desori can have a pronounced notch in the blade, but desori teeth are usually narrower then the one you have.

After looking up these sharks I was able to find this image that appears to be very similar to the one I found.images.jpeg.74c0b2d89ba4ddb873436820a5c951ce.jpeg

 

 it is a bit hard to tell with the root being worn, but it does look right.
