piranha
This is an undescribed species according to the comments from a Phacopinae specialist friend: "Whether or not the eye "formula" is relevant to exclude Reedops, it is currently difficult to say. We cannot exclude that paedomorphic species have less developed eyes in the number of lenses than typical Reedops. I would exclude Reedops because of the following reasons: Lateral anterior border furrow of the cephalon as good as missing, median portion of L1 completely flat (due to too much prepping???), posterior margin of eye rather close to posterior border furrow, low number of lenses in a not so small individual, thoracic distal axial lobes not developed, pygidial pleural fields largely smooth. Boeckops appears to be closer, see Chlupáč 1977, especially Boeckops zenonis. Chlupáč wrote that smaller specimens have less lenses than large ones. However, like Reedops, Boeckops zenonis has well-developed thoracic distal axial lobes! Though it remains enigmatic, I would support an assignment to a Boeckops-like form rather than to Reedops."
