Lodgepole Formation, Kinderhookian Lower Mississippian (within basal 3 foot contact with underlying Devonian Three Forks Fm. Centenial Range, Fremont County, Idaho As far as I know it is the only known complete specimen in existence. Pretty incredible bug! It is about 1cm long with a matrix of 5.5 x 8.5cm Eofallotaspis sp.
